If your pet has been seen, by a Veterinary Surgeon, in a consultation at Hayle Veterinary Surgery we may be able to issue a written prescription which can then be used to purchase the prescription drug from an online pharmacy.
Please fill in ALL the fields below and we will contact you by phone or email if we have any issues in supplying a written prescription. All prescriptions are sent via email directly to the pharmacy of your choice.
Each prescription drug requires an individual written prescription which will be chargeable. Written prescriptions must be paid for before they are sent to the pharmacy - please phone the surgery to find our up to date price before you complete the form below.
PLEASE NOTE We cannot process the request unless we have an order number from the pharmacy. You MUST ensure that each item you are ordering online is in stock at the pharmacy otherwise additional fees may apply.
Written prescriptions for 'Prescription Only' flea and worming control can only be requested at the time of a consultation with a Veterinary Surgeon.
Please take care in completing the form as any inaccurate details completed below may delay your prescription from being sent.

All fees MUST be paid for at the time of consultation or discharge after treatment. Payment can be made by Cash Delta, Switch, Visa or Access.
Please do not send children under 16 yrs with pets to the suregry unless by prior arrangement with the Veterinary Surgeon. Please do not send third parties with pets, as treatments can be complicated by lack of accurate history.
For safety reasons, please ensure that your pet is adequately restrained whilst on the premises – dogs should be on a lead and cats, rabbits, birds etc in a suitable carrier.
